Cervical Radiculopathy: Treating a Pinched Nerve in the Neck

There are numerous treatments for a pinched spinal nerve in the neck. However, rigorous evidence of their benefit is scant.

Let’s suppose that you have been diagnosed as having a pinched nerve in your neck, also known as cervical radiculopathy. If so, you probably have pain in the neck and one shoulder. The pain might radiate into your arm and you might have weakness or numbness in the arm as well. Moving your neck in certain positions probably worsens the pain.

If you’re a younger adult, the pinch could be due to a herniated (slipped) disc. Discs are the soft spacers that separate each pair of stacked neck-bones (vertebrae). If you’re an older adult, the pinch is more likely due to a bony spur (spondylosis). In either case, you’re in good company. A survey in Sicily showed 3.5 active cases at any one time of cervical radiculopathy per population of 100,000. In Rochester, Minnesota, another survey showed 85 new cases each year of cervical radiculopathy per population of 100,000.

Let’s say that your doctor has evaluated you thoroughly by taking a history of your symptoms and performing a physical examination. Perhaps with the additional help of an MRI of your cervical spine (neck) and electrical tests of nerve and muscle function (nerve conduction studies and electromyography) the diagnosis of cervical radiculopathy is deemed definite. Furthermore, there is no sign that the spinal cord itself is pinched. Now what?

Now what, indeed. Choosing a treatment for this condition is far from straightforward. Out of hundreds of published medical reports concerning treatment of cervical radiculopathy, most are case reports or case series. A “case series” translates roughly as: “We gave six patients in a row the same treatment and five of them got better.” What can be concluded from a study of this kind? Did the treatment make the patients better or would they have improved anyway? We don’t know.

The missing ingredient here is a comparison group of untreated or differently treated individuals known as a control group. The other mark of a quality study is that the chosen treatment is randomized, meaning that the research subjects agreed in advance to be assigned to one treatment group or another based on the equivalent of a coin-toss. So out of the hundreds of published studies involving treatment of this common condition, how many were randomized controlled trials? Unfortunately, the answer is just one.

Liselott Persson, Carl-Axel Carlsson and Jane Carlsson at the University Hospital of Lund, Sweden, randomly allocated 81 patients who had symptoms of cervical radiculopathy present for at least three months to any of three treatments — surgery, physical therapy or a cervical collar. The patients ranged from 28 to 64 years old and 54% of them were male. The surgeons used the so-called Cloward procedure, removing fragments of protruding discs and spurs through an incision in the front of the neck, and then fusing two neck-bones together by means of a bone-graft. Physical therapy involved 15 sessions over a span of three months and consisted of whatever the physical therapist considered appropriate, variously including any of the following: heat application, cold application, electrical stimulation, ultrasound, massage, manipulation, exercise and education. In the cervical collar group, patients wore rigid, shoulder-resting collars every day for three months. Additionally, some of the subjects wore soft collars overnight.

How did the study turn out? Three of the subjects who were assigned to surgery refused the procedure because they had already improved on their own. For statistical purposes their outcomes were included with those who actually received the operation. After three months the surgery and physical therapy groups reported, on average, less pain. After an additional 12 months patients in all three groups had less pain than at the beginning of the study and the outcomes of each treatment were statistically alike. Measurements of mood and overall function following treatment were likewise equal among the groups.

So, over the long haul, no treatment was better than the others. Of course, within each group some patients did better or worse than others and this spread of outcomes was not reflected in the overall averages. In fact, five patients in the collar group and one patient in the physical therapy group went on to receive surgery owing to lack of satisfactory improvement. In addition, eight patients in the surgery group underwent a second operation that in one case was due to a complication of the first operation.

With this Swedish study representing the only rigorous investigation of treatment outcomes in cervical radiculopathy, there are a number of unanswered questions. For example, what are the effects on cervical radiculopathy of painkillers, anti-inflammatory drugs, local injections, systematic traction or other forms of surgery? We don’t know. What happens if there is no treatment whatsoever? We don’t know the answer to that question either.

Thus, in the care of individual patients there is a yin-yang balancing act between the medical edict of “Above all, do no harm” and the practical dictum of “Do what you have to do.” This balancing act usually means starting with less intrusive treatments like drugs and physical therapy. If symptoms fail to improve or become unbearable, an operation may be helpful.

Carpal Tunnel Syndrome: Pinched Median Nerve at the Wrist

Carpal tunnel syndrome, a source of annoying symptoms in the wrist and hand, can be reliably diagnosed with the help of nerve conduction studies. Surgical and non-surgical treatments of proven effectiveness are available for this pinched-nerve condition.

Carpal tunnel syndrome is by far the most common and widely known of the “pinched nerve” conditions. This article addresses: What is it? Who is at risk for this condition? How is it diagnosed? What kinds of treatments work best?

Carpal tunnel syndrome refers to symptoms caused by entrapment of the median nerve in the carpal tunnel. “Carpal” itself means “wrist,” so a carpal tunnel is nothing more than a wrist tunnel. This particular tunnel can be a crowded place, as it contains not just the median nerve, but nine tendons as well. The “syndrome” consists of some combination of pain, numbness and weakness.

Pain, numbness, or both, are the usual earliest symptoms of carpal tunnel syndrome. Pain can affect the fingers, hand, wrist and forearm, but not usually the upper arm or shoulder. Numbness affects the palm side of the thumb and fingers, but usually spares the little finger because it’s connected to a different nerve.

When weakness is present, it usually indicates that the condition is already severe, and when muscles atrophy (wither) it means the condition is even worse. The affected muscles are those downstream from where the nerve is pinched, and can include those controlling any of three motions of the thumb. In addition, bending of the first knuckles of the index and middle fingers can be affected, as can straightening of the second knuckles of the same fingers. When muscle atrophy is present, it is most evident in the muscular ball at the base of the thumb.

Carpal tunnel syndrome occurs more frequently in women than in men. People who work with their hands a lot – for example to sew, operate hand-tools or perform assembly-line work – are at increased risk for developing this condition. Various medical conditions can also increase the risk of carpal tunnel syndrome, including injuries, arthritis, diabetes, low levels of thyroid hormone and pregnancy. In the case of pregnancy, carpal tunnel syndrome often appears in the third trimester and resolves after the woman delivers.

Optimum diagnosis of this condition combines the time-honored methods of a doctor’s history-taking and physical examination with tests of nerve function called nerve conduction studies. Nerve conduction studies are exquisitely sensitive in detecting impairment of the median nerve at the wrist, particularly when the median nerve is compared with a nearby healthy nerve in the same patient.

In nerve conduction studies, the nerve on one side of the carpal tunnel is activated by a small shock to the skin. An oscilloscope measures how long it takes for the resulting nerve-impulse to arrive on the other side of the carpal tunnel. When the median nerve is pinched, the nerve-impulse is delayed or blocked. Nerve conduction studies are so sensitive that sometimes they show problems that aren’t even causing symptoms. That’s why nerve conduction studies don’t stand alone in diagnosing carpal tunnel syndrome. The examining physician needs to decide if the results make sense for the particular patient in question.

Nerve conduction studies not only show whether or not the median nerve is impaired at the wrist, but also provide precise data concerning how bad the impairment is. In addition, these studies survey the function of other nerves in the arm and hand. Occasionally, a nerve in an adjacent tunnel (the ulnar nerve in Guyon’s canal) can also be pinched. In other cases, nerve conduction studies show that the problem is not one of single nerve-pinches, but rather a more diffuse pattern of nerve-impairment called polyneuropathy. Of course, sometimes the studies are completely normal and suggest that the symptoms are due to something else.

To treat carpal tunnel syndrome, starting with “conservative” treatment makes sense in most cases, especially when the symptoms are still in the mild-to-moderate range. Conservative treatment usually includes a wrist-splint that holds the wrist in a neutral position. In a study published in 2005 researchers at the University of Michigan investigated the effectiveness of wrist-splinting for carpal tunnel syndrome in workers at a Midwestern auto plant. In a randomized, controlled trial – the gold standard method for judging treatments – about half the workers received customized wrist-splints that they wore at night for six weeks. The remaining workers received education about safe workplace procedures, but no splints. After treatment the workers with splints had less pain than those without, and the difference in outcome was still evident after one year.

Conservative treatment might additionally include use of anti-inflammatory medications like aspirin or naproxen, or even steroid drugs. A more intrusive, though still non-surgical, treatment consists of injecting steroid medication into the carpal tunnel itself. This might benefit selected patients, but in a 2005 randomized, controlled study of patients with mild-to-moderate symptoms, researchers at Mersin University in Turkey showed that patients receiving splints did better than those who received steroid injections.

Surgeons can relieve pressure on a pinched median nerve by cutting a constricting, overlying band of tissue. A 2002 study at Vrije University in Amsterdam compared surgical treatment to six weeks of wrist-splinting. After 18 months 90% of the operated patients had a successful outcome compared with 75% in the splinted group.

In some cases it can be reasonable to try conservative treatments without first confirming the diagnosis with nerve conduction studies. However, in the author’s opinion, this risk-free form of testing should be performed prior to any carpal tunnel surgery. (Full disclosure: The author performs nerve conduction studies!)

I like to buy the 12 ounce bottles of water and keep them in the fridge. Whenever I go to the gym I grab a bottle. On my way out the door to work, I grab a bottle. Before a trip that will be 30 minutes or longer, I grab a bottle. When I watch a movie, I grab a bottle.

Whenever I am sitting watching TV or a movie, I will keep the bottle in my hand with the lid off. I take a lot of little sips until the bottle is gone. A great way of making sure you drink more water is to make sure it is easily accessible.

2. Keep it cold.

I don’t know many people who like to drink room temperature water, I sure don’t. If you keep your water cold, you will drink more. I make sure there is always ice in my freezer, this make it very easy to have a cold glass of water anytime.

3. Chose water in restaurants.

Whenever I go out to eat, I always drink water with some lemon. The lemon adds a little spunk to the taste and the water is always nice, cold and refreshing. After a short time you will stop craving your old drink of choice and will begin to appreciate the taste, or lack of taste, that water provides.

Give water a shot and your body will be thankful!

The radial of contact lenses is the concentric design lens. The inner lens will work on either the nearness vision or the distant vision and so can the outer part of the lens.

Translating design contacts are similar to bifocal eyeglasses where the distant correction is above the nearness vision correction. A line makes the lenses separate. The bottom of the lens is flat to keep it from moving around in your eye when you blink.

Both distant and near vision are located at the center of the Asferic Design lenses. The near correction in the center is surrounded by distance correction.
Sometimes they can be reversed in some situations. You and your doctor can decide that.

With mono-vision design lenses you have one power lens in one eye and then the other power lens in the other eye. Usually the distant vision lens is worn in the dominant eye. An examination by your doctor will be able to determine this.

There are also simultaneous vision lenses. Your eyes can focus on things that are both near and far at the same time. Distant and near correction is concentric rings.The near and far parts of the lens are in sight all the time so the light from both distant and near objects can be focused on at the same time.

Simultaneous vision contact lenses have a problem. The light from the near part of the lens will go through the distant part and vice versa when the eye is looking through it. Both in focus and out of focus can be taken in by the eye at the same time.

The brain has to figure out which is the correct image. Whether contact lenses are bifocals or not, you must obtain a prescription. Your eye doctor will do a very thorough exam to decide if you are a candidate for bifocal contacts and what type might be right for you.

Lenses that fit and are comfortable may take time to find, as with any contact lens there is an adjustment. Bifocal lenses may not be for you, at least not in the contact lenses currently available.

There may be contact lenses out there, don’t give up.Do your research if you really want contacts and need bifocals and keep informed on the types of lenses available to you.There are resources available online, through your eye care physician, and in some cases right from the manufacturer.

There are discount websites for contact lenses and some manufacturers will give you a coupon for their lenses if you try them. Your budget will affect your decision about bifocal lenses without a doubt.

Check with friends and family who wear contacts and see what their experience was like. Although not everyone has the same experience, the information could help you and your doctor make the decision. Bifocal contact lenses are available now for people with astigmatism.

Proper skin care is one of those things that more and more people are embracing nowadays, particularly because of the media campaign that subconsciously pushes people to be more beautiful. However, as with anything of this sort, not all the information one needs to know is actually known by the general public. In such situations, the general public tends to develop “knowledge” of its own, which are easily dismissed as myths and urban legends by medical experts. Here are a few of the more common myths.

Myth #1: Eating Some Foods Can Cause Skin Infections

Skin infections are not caused by the food you eat. No, acne is not caused or affected by your diet either. Your food intake has no effect on whether or not your skin can become a target for acne, pimples, or any other skin infections. The fact is, your diet has minimal effect on your skin. Instead of watching what you eat, you might want to try cutting down on the stress and anxiety to minimize the chances of your skin getting blemished. The only way food can have an appreciable effect on your skin is if you have an allergy to the food eaten.

Myth #2: Facial Exercises Make The Skin Look Younger

There is absolutely, positively no truth to this. The harsh reality is that doing facial exercises will do more harm than good. Your skin is elastic, but only to a certain extent. Like all things that are elastic, there are limitations to just how much you can pull before breaking it or deforming it. Similar to a rubber band, if you pull at it enough, you’ll eventually deform it. In the case of the skin, do enough exercises and this’ll just make those wrinkles and lines more visible.

Myth #3: A Good Tan Can Kill Bacteria

This one is little more than scientific speculation at the moment. Ultraviolet light may have an effect on bacteria or it may not, science isn’t really sure about that right now. However, even if UV rays did have an effect on bacteria, there’s no indication that it would have any appreciable effect on bacteria beneath the surface of the skin, which is where most skin infections start out. Sure, tanning might be a good way to relieve stress and anxiety, but the possibility you might end up getting skin cancer is something you should keep in mind before you lounge all day on the beach.

Myth #4: Too Much Drink Can Make The Skin Blow Up

There is some scientific basis for this one, as alcohol tends to open up the pores and make a person look more flushed. However, alcohol cannot make the skin blow up and have acne appear all over your skin overnight. The open pores might make it easier for dirt to get into them and aggravate them if they are already clogged, but it is unlikely to increase the damage that was already there by any significant margin.

Myth #5: Dirt Causes Acne

To confirm: dirt clogged in the pores does not cause acne. It can cause problems, definitely, but it cannot cause acne, so don’t grab an over-the-counter acne treatment to help alleviate them. An exfoliant is more likely to help if dirt has clogged your pores, as that sort of treatment will peel away layers of the skin, eventually taking the dirt along with it. While acne treatments based on salicylic acid might help, this is because the acid is an exfoliant and not because acne problems are the same as having dirt in your pores.

What is Chronic Fatigue?
The basic definition of Chronic Fatigue Syndrome does not make sufferers feel much better. There is no known cause for the fatigue that is present in an individual that has no relation to over-exertion and is not rectified by rest.

Frequently Chronic Fatigue Syndrome begins during periods when individuals have been sick, under a tremendous amount of stress, or for no apparent reason at all. Many individuals have noted that after bouts with the flu, mono, a cold, bronchitis, hepatitis, and other similar illnesses CFS arises. Unfortunately, Chronic Fatigue Syndrome persists for months, sometimes coming and going and other times constantly affecting the patient. Other illnesses begin to clear up after a few days or weeks, but Chronic Fatigue Syndrome sticks around with no clear cause.

Diagnosing Chronic Fatigue
